The working group met in Madrid to discuss the introduction and application of molecular karyotyping in a diagnostic setting. The aim of the working group meeting around molecular karyotyping is (1) to address the questions to be answered to bring array CGH from the scientific environment to the clinical diagnostic laboratories (2) to be a platform for dissem­inating information relevant to the use of array CGH in a clinical setting and (3) to be a discussion forum among users of array CGH technology. The meeting was attended by a large audience.

First, Joris Vermeesch provided an overview of the different technical and biological questions that need to be addressed for the optimal introduction of this technology in the clinical environment. In short, three main issues were identified: (1) There are a series of technical questions, relevant to the diagnostic applica­tion of the technology, such as appropriate quality criteria, need for reference material and quality control materials, how to validate the array CGH findings, which array provider to choose, etc. (2) A series of biological questions need to be addressed.  The benign variations within the human genome need to be mapped, clinically relevant variation needs to be collected in order to associate novel chromosomal imbalances with clinical phenotypes. (3) Several laboratory-technical issues need to be addressed such as novel reporting criteria, novel ISCN nomenclature, and which data needs to be reported, recorded and stored. It was decided that within the ECA website, a forum will be created to address these questions.  On the one hand, the website will collect all relevant information surrounding array CGH. This website will host links to the website addressing the different issues.  Information on any of the above issues can be provided to the working group coordinators and will be further distributed on the website.

Philippos Patsalis presented the results of a survey amongst array CGH providers. Within his survey, he addressed a series of questions relevant to array CGH users. The survey was sent to 10 companies providing different array formats and replies were received from 5 companies. The answers of the companies to the survey can be seen at the ECA website. Users of the different array platforms can add personal comments on the use, quality, service, etc. to the benefit of the community. Comments can be sent to Mariano Rocchi and will be attached to the survey forms.

Last but not least, the term ‘molecular karyotyping’ to indicate genome-wide CGH was challenged. A number of attendees of the working group argued that the term ‘molecular karyotyping’ was not an accurate terminology since array CGH does not look at the chromosomes of a single cell, cannot identify translo­cations or the degree of ploidy. On the other hand, the terminology has already a high degree of acceptance.

The meeting of the PWG on Cancer cytogenetics, solid tumor studies, chaired by L.Larizza, was held from 03:00 to 04:00 PM on Saturday 4 June 2005.

It comprised two talks both focussing on carcinomas, which as with other solid tumors raise problems of karyotypic analysis and interpretation, and require powerful technological approach(s) to permit detailed and thorough investigations.

The first lecture was given by P. Edwards (Cambridge) on 8p11 translocations in breast cancer. For many years the group of P. Edwards in Cambridge has run a project aimed at characterising breakpoints underlying recurrent chromosomal rearrangements in breast carcinoma, which might pinpoint genes of pathogenetic relevance. This particularly applies to 8p11 translocations, several of which have been characterized at the molecular level using the FISH and FISH-related techniques available for this purpose. Interestingly, this research project has been enhanced through collaborations with other groups, both in Cambridge (C.Caldas) and in France (M. Chaffanet), working actively on the same genomic region in breast cancer. Through a comprehensive overview, a series of unique 8p breakpoints were described, and their refined mapping compared to the location of the minimal amplicon detected in breast cancer by array-CGH. Genes with a putative function consistent with an oncogenic activity were revealed. Their role in carcinoma development and progression is under test.

The second overview was provided by J. Squire (Toronto) on PTEN microdeletions in prostate cancer. The epidemiological impact of prostate cancer on men’s health is comparable to that of breast cancer in women. The main issue of J. Squire’s lecture concerned the alterations which mark the transition to aggressive prostate cancer. A critical marker in this respect is PTEN (phosphatase and tensin homolog), microdeletion of which is detected in prostate cancer, but not in prostatic intra-epithelial neoplasia, by FISH using tissue microarrays. Interestingly a correlation was observed between deletion status and immunohistochemical PTEN antibody analysis. By contrast, other PTEN lesions, including loss of heterozygosity or homozygous deletions, point mutations and promoter hypermethylation, which are frequently detected in prostate cancer, are not as specific as PTEN microdeletion for the transition from benign hyperplasia to carcinoma. The relationship between genomic alterations of PTEN and prostatic neoplasia was also discussed in the context of the high frequency of duplicons flanking PTEN gene locus on 10q23, that may mediate PTEN deletion.

Both lectures covered broad issues and stimulated lively discussion which were further expanded in small expert groups

The PWG meeting took place in conjunction with the meeting of the workpackage 11 Cytogenetics of the European LeukemiaNet. The presentation of the ELN work package 11 Cytogenetics and possibilities of a future cooperation between ECA and ELN were the main topics. Over 60 cytogeneticists attended the meeting. The session was chaired by Bertil Johansson.
The role of cytogenetics within the scope of the European LeukemiaNet

By presenting the European LeukemiaNet (ELN, http://www.leukemia-net.org) it must be underlined that this project offers the possibility to multiple cooperations with the European Cytogeneticists Association.

Which are the objectives of the ELN?

Leukemia research and therapy have achieved high standards and even a leading position in several European countries with regard to clinical trials and diagnostics. A true European world leadership, how-ever, has not been accomplished yet due to national fragmentation of leukaemia trial groups, diagnostic approaches and treatment research activities.

The objective is to integrate 78 leading leukaemia trial groups, their 83 interdisciplinary partners and industry across Europe to form a cooperative network for advancements in leukaemia-related research and health care. Integration will be supported by central information, communication, education and management structures. Moreover, it will enable the creation and integration of clinical, cytogenetic and molecular genetic databases.

Clinically oriented goals are to intensify target and drug discovery, to shorten the time period to clinical translation, to apply advanced genomics, telematics and biotechnology to therapeutic progress and the introduction of standards for diagnostics and treatment.

Leukaemia serves as a model for diagnosis and therapy of malignancies. The value of elucidating the cytogenetic and molecular genetic defect of a leukaemia is highlighted by the fact that the first effective molecularly targeted therapies have been developed for leukemias, for example Imatinib in chronic myeloid leukemia and all-transretinoic acid in acute promyelocytic leukaemia.

To further explore this avenue of research, a multidisciplinary team approach is needed, involving scientists in various fields of leukaemia research, i.e. drug discovery, leukaemia diagnostics, cytogenetics, molecular genetics, clinical trials methodology and biostatistics.

A further goal of the ELN represents epidemiological research. Limited information is available regarding the potential impact of environmental exposures or gene and environment interactions. We know that leukaemia-associated chromosome aberrations show a geographic heterogeneity due to ethnic, genetic, socioeconomic and environmental factors. The analysis of the geographically heterogeneous distribution of chromosome aberrations and a correla-tion with morphologic, immunologic, molecular genetic and treatment data could lead to the identification of local leukaemogenic factors in different parts of Europe.

In order to achieve all these objectives the European LeukemiaNet has a specific structure. In the middle of this network are central services, the network management center, the European leukaemia information center and the central information and communication services. With these central services connected are the clinical trials, the diagnosis and follow-up platform, the treatment research groups and the platform concerning registry and education (Figure 1).

The workpackage Cytogenetics is part of the platform diagnosis that includes four workpackages, who cooperate jointly and also with other workpackages, for example of the platform clinical trials.

The integration of all major cytogenetic laboratories in a European cytogenetic platform to analyse large data sets of leukaemia-associated chromosome aberrations across Europe is one of the objectives of the workpackage Cytogenetics. The cytogenetic data of all newly diagnosed patients treated within clinical trials will be entered in a newly established data base and correlated with clinical and other diagnostic data in order to improve prognostic evaluation.

Further objectives concern the standardization of cytogenetic and molecular cytogenetic techniques in order to improve the quality of cytogenetic results across Europe. Metaanalyses have to be performed, consensus reports and expert reviews must be published and guidelines have to be developed on a European scale.

Each of the 18 workpackages of the ELN accepted tasks that are differentiated in deliverables and milestones. Milestones are fundamental institutions that enable the performance of deliverables and are joined with them.

The first milestones that we have to overcome concern the establishment of information and communication structures. That became possible by the establishment of the ELN homepage and by the organization of communication structures within the workpackage (www.leukemia-net.org).

The creation of a European platform for leukaemia cytogenetics had been initiated. The cytogenetics platform will contain joint research activities, proce-dures for quality control and specific links to relevant resources, to meetings and to announcements of jobs. The cytogenetics platform is a work in progress.

A further milestone, also set about last year, concerns the presentation of difficult cases and the provision of additional methods for further investigations. A discussion forum was integrated into the ELN internet presentation for this purpose.

The management of the workpackage is devoted to Christa Fonatsch, Claudia Schoch and Harald Rieder, as lead participants. The steering committee consists of the three lead participants and of representatives of different countries: Dr. Beverloo (NL), Prof. Dastugue (F), Prof. Hagemeijer (B), Prof. Jotterand (CH), Prof. Knuutila (SF), Dr. Lillington (GB) and Prof. Mecucci (I).

After the presentation of milestones we switch over to the deliverables. One of the deliverables is the holding of workpackage meetings.

In the meantime three meetings took place.

The first meeting was dedicated to tasks and concepts of our workpackage and was very important to become acquainted with colleagues and to discuss joint research activities.

The second meeting concerned the standardization of cytogenetic methods. After this meeting Claudia Schoch prepared a draft of consensus protocols for different leukemias. A number of participants of our workpackage supplemented this draft and now a consensus protocol has been compiled that should be published in Genes, Chromosomes and Cancer.

During the third meeting first results of joint research projects have been reported, such as the 11q amplification project, presented by Dr. Zatkova (Vienna), a project of Prof. Mecucci (Perugia) concerning NUP98 fusion partners and a MYC amplification  project of Dr. Storlazzi (Bari).

Prof. Hagemeijer called for cooperation in a T-ALL project, concerning CAN-ABL fusions and Dr. Huret appealed for rare abnormalities in order to publish them in his Atlas of Chromosomes in Cancer (http://www.infobiogen.fr/services/chromcancer).

Prof. Knuutila presented the role of array CGH to screen gene amplifications and Dr. Lillington sug-gested to integrate SNP array data to analyze AML with normal karyotype.

Upcoming activities and deliverables of the next months:

1.     A European registry for leukemia cyto­genetics has to be established.

    A platform of data exchange with other workpackages will be established.
    For the identification of new recurring chromosome aberrations a prototype cytogenetic online research data base (cydas.org) was created by Prof. H. Rieder and Dr. B. Hiller and made available to the ELN. New recurring chromosome aberra-tions should be entered in the Atlas of Chromosomes and Cancer of Dr. Huret.
    Additional methods to clarify unidentified karyotypes will be provided (24-color FISH, CGH, array CGH, subtelomere FISH, etc).
    Gene amplification analyses will be per-formed.
    A software for analysis of large cytogenetic data sets in correlation with clinical data and other diagnostic information will be devel-oped.
    Spread of excellence and knowledge could be achieved by cooperation with the ECA and by cooperation with other workpackages. In cooperation with the ECA educational and training programs could be elaborated and started.
    The important topic “quality control” has to be commenced.
    In cooperation with companies new molec-ular cytogenetic probes should be generated and evaluated. 

Basis and possibilities of a future cooperation between ECA and ELN (presented by Harald Rieder)

During the joint ECA PWG and ELN WP meeting a common interest of the ELN work package Cyto-genetics and of the ECA was emphasized to improve the quality of leukemia cytogenetic diagnostics in health care and research, to enhance public relations and to foster cooperations with companies. Joint activities of the ELN work package 11 were discussed with regard to the harmonization of cytogenetic techniques, the definition of minimal standards in leukemia cytogenetics, the definition of standards in quality control and the improvement of exchange and scientific usage of cytogenetic data. To achieve a fruitful cooperation the joint development of consensus protocols and guidelines and the organi-zation of concerted workshops were proposed. As current topics of forthcoming workshops theoretical and practical training in disease specific methods, cytogenetic data base management and data mining, and quality control in leukemia cytogenetics were suggested. The participants suggested to establish a platform for the exchange of cytogenetic laboratory protocols which will be included in the ELN WP11 web site.

 The well-attended Prenatal Diagnosis Working Group session posed the question “Is FISH/PCR testing sufficient for ‘raised Down Syndrome risk’ referrals” with speakers from 5 different countries talking on specific aspects and summarising their national perspectives. Sia Polihronis, from Sydney, indicated that in Australia patient access to screening is very uneven, and that karyotyping is compulsory for all referrals and is always provided. Rolf Wegner (Berlin) described a similar requirement for karyotyping in Germany, with FISH/PCR being specifically excluded from Health Service funding. It is, however, covered by private health insurance and can be paid for on an individual basis. Vincent Cirigliano (Barcelona) presented experience with the widespread use of QF-PCR for rapid aneuploidy screening in Spain. Data from 28,000 procedures suggests a very high success rate, with an overall detection of 95% of significant cytogenetic abnormalities. As the missed abnor-malities were concentrated in the group with abnormalities detected on ultrasound, he proposed that for ‘advanced maternal age’ and ‘biochemical screening’ referrals, a policy of QF-PCR without karyotyping would be the most cost/resource effective. Brigitte Faas from the Netherlands described her experience with the use of MLPA as an alternative to QF-PCR based on 4,500 cases. She provided evidence that it is not only effective and accurate, but that it clearly has wider potential for abnormalities not detected by QF-PCR. The ability to detect 60-70% mosaicism for trisomy 21 was shown. There is no current agreed policy for rapid aneuploidy screen in The Netherlands. The UK situation was covered by Tony Parkin (Nottingham). Here, a National Screening Committee, which decides funding policies, is looking specifically at trisomy 21 screening. Their draft proposal that for trisomy 21 screening FISH or QF-PCR is sufficient, is at odds with the data derived from a large ACC retrospective study of what would be missed if such a policy was implemented nationwide. The results of this UK study have been accepted for publication in Lancet, and will contribute to the continuing debate.

A workshop of the PWG was held on Saturday 5th of June with 50 participants.

The coordinators, J.P. Bureau and J.M. Garcia-Sagredo, explained the major aim of the PWG: to facilitate the creation of a network for all concerns and problems of E.C.A. members interested in mutagenesis.

The main activity of most ECA members working in the field of mutagenesis lies in clinical cytogenetics rather than cytogenetic toxicology. This creates difficulties in obtaining grants, in coordinating their research with current or upcoming European projects, and in obtaining funds for training in cytogenetic toxicology. Furthermore, an information network is of special interest for colleagues from countries where important health concerns arise from environmental pollution.

A similar situation existed at the Bologna PWG meeting, but the activities proposed since that meeting were insufficient. The coordinators invited ideas and proposals, including proposals for new coordinators for this permanent working group. The outcome of an intense and interesting discussion was that the existence of the PWG and the creation of a network to be coordinated with existing networks in other associations such as the EEMS is still necessary.

A PWG meeting on the clinical and molecular approach to cytogenetic syndromes was organised by ECARUCA (European Cytogeneticists Association Register on Rare Unbalanced Chromosome Aberrations), and was chaired by Albert Schinzel and Conny van Ravenswaaij.

The meeting was well attended, with around 50 participants. During the meeting, different members of the ECARUCA team updated the audience on the project’s progress and the potential for the database.

Since the start of the project in 2003, we have set up a website (www.ECARUCA.net) and an online database for the collection of cytogenetic, molecular and clinical data from patients with rare unbalanced chromosome aberrations. Currently, around 400 professionals within the field of human genetics hold an account. With this account, members can query the database and submit information regarding new cases. The practical steps regarding how to do this, were explained by David Koolen and Ilse Feenstra.

Mariluce Riegel gave an update on the availability of assistance with cytogenetic investigations of patient samples for those laboratories with limited facilities to analyse exact breakpoints. Up to the 1st of January 2005, 57 patient samples have been analyzed in the Institute of Medical Genetics in Zurich. In some of them, the previous determination of aneuploid segments had to be significantly revised.

Participants discussed several points concerning the two-step model for the entry of cytogenetic and clinical data. First, cytogenetic data needs to be entered, preferably by a cytogeneticist. After checking by the Clinical Database Manager, the clinician involved will be contacted for the submission of clinical data. We would prefer that clinical data is entered by a clinical geneticist, in order to collect the highest quality description of any dysmorphic features that are present.

All National Coordinators have been asked to encourage their colleagues to submit additional cases of unbalanced chromosome aberrations. It is hoped that each country can submit at least 25 new cases per year.

If a centre wishes to submit a significant number of patients where data is already in place, it may be possible for the Clinical Database Manager of ECARUCA to visit the institute. Using a translator (if needed) the information regarding these patients can then be entered into the database directly by the Clinical Database Manager.

In the near future, ECARUCA will set up a monthly email for all account holders, including a list of all cases submitted to the ECARUCA database in the past month. This will include the name of the referring centre, so that other account holders can contact that centre. This will increase the involvement of account holders with ECARUCA.

We were very pleased that a number of people introduced themselves after the meeting and spontaneously offered to become a National Coordinator. Welcome to you all and we look forward to a fruitful collaboration!

The PWG is taking part in the work package devoted to quality assessment in cytogenetics within Unit 1 of EUROGENTEST (www.eurogentest. org), an EU 6th Framework funded Network of Excellence. This work package is headed by Ros Hastings from Oxford, UK. She has also joined this PWG as a co-ordinator. In collaboration with the Eurogentest the PWG members prepared the draft of the “ECA Cytogenetic Guidelines and Quality Assurance”.

The PWG meeting at the 5th ECC was entirely dedicated to the presentation of this first revised version of the guidelines. This document aims to establish common guidelines and standards to use as a reference manual in all European countries, especially in those without national regulations, and assist in developing national standards. The guidelines were created by taking into account and comparing existing quality assessment schemes and good laboratory practice documents from different countries. The guidelines include aspects of quality control and assurance for most of the routine methods employed by cytogenetic laboratories. The format has been divided in two main areas:

- GUIDELINES that include recommendations on staff, workload, equipment, laboratory procedures, reporting, records and storage;

- QUALITY ASSURANCE that covers confidential­ity, lab staff education and training, internal and external quality assessment schemes, health and safety and accreditation.

The revised draft is currently published on the website at the address: http://www.biologia.uniba.it/eca/.

Everyone is invited to read the document and send us comments since the document still remains open for discussion. The PWG would especially appreciate your opinions on the opportunity to introduce:

- reason for referrals to cytogenetic analysis (prenatal, constitutional, FISH studies, cancer cytogenetics),

- details on minimum standards, mosaicism detection, minimum success rates, report times,

- advice on technical aspects, storage (slides/images, time),

- procedures for checking analysis,

- information on preparing for accreditation.

 Comments will be collected until the middle of September when the PWG members intend to meet to make amendments. The new reviewed version will be again on line for comments after the meeting. The guidelines will be put forward to the ECA board for recognition and for endorsement of the membership at the general assembly in 2006.
